Becoming a Client PDF Print E-mail

The Enrollment Process

If you are interested in becoming a client of CARES, call our office and ask for a new client intake appointment. After that appointment, an orientation class is conducted where you will learn about HIV and all the services at CARES.

When you come to your intake appointment, you will need to bring the following items with you. Eligibility for services is based on federal regulations including HIV status, income and residence.
  1. Proof of where you live. A copy of a utility bill, lease agreement, rent receipt or other documentation will be enough.
  2. Documentation of your household income for the past 6 months.
  3. Documentation of HIV status.
  4. Photo identification such as a passport, drivers license, school or work identification card.
If you don’t have all of this information for our intake, don’t worry. We can help you find the paperwork you need. We will not deny services to you if you don’t have all the documents listed above.

Paying for Services

If you do not have insurance, you will need to work with our eligibility staff to find a program for which you are eligible. There are several ways that your care can be covered. If you have private insurance and CARES accepts that insurance company, your medical visits may be covered. CARES also accepts Medicare and Medi-cal.

Making an Appointment

To request an appointment for new client enrollment call 916.914.6233 and ask for an intake appointment.

After regular business hours, CARES provides our clients with urgent medical needs access to a physician on call. For non-urgent problems, we ask that you call our office the next business day to request an appointment.

You can also leave a voicemail for the nurse after hours at 916.914.6215, and the nurse will return your call the next business day.
